This assessment task is scheduled to take approximately three (3) hours.

Refer to the due date in the Unit Plan. 
What to submit

Your completed observation checklist (including assessor and student signatures).

How to submit

 Scan your completed observation checklist and documentation and save it as a PDF file using the naming protocol provided below. HLTWHS001_AT3_your name

Upload your submission to the assessment within your LMS

How to present your work

Ensure you have labelled it with your name and Student ID, completed the assessment task cover sheet and signed all required fields at the bottom of each observation task checklist.

Context and conditions of assessment

HLTWHS001 - Participate in workplace health and safety

Skills must be demonstrated:

- in the workplace

OR

- in an environment that provides realistic in-depth industry validated scenarios and simulations to assess candidates’ skills and knowledge.

Assessment must ensure use of: 

- current workplace policies and procedures for WHS

- PPE relevant to the workplace and job role of the worker

In addition, assessors must satisfy the Standards for Registered

Training Organisations (RTOs) 2015/AQTF mandatory competency requirements for assessors

Where will this assessment take place This assessment task will take place on campus in a simulated workplace environment. 
How many attempts at assessment are permitted

You will be given three (3) attempts to satisfactorily (S) complete this assessment.

All criteria must be demonstrated satisfactorily. If you do not correctly demonstrate each criterion you will receive a grade of Not Yet Satisfactory (NS). 

If you receive a grade of Not Yet Satisfactory (NS), you will be given a further two (2) attempts to perform the online assessment task satisfactorily.

If you have a disability and believe you are eligible for alternative or adjusted assessment procedures, please discuss this with your Trainer/Assessor.

 Full BHI uniform for assessment & Laboratory assessments. closed toe shoes, PPE

 Simulation laboratory, all equipment for evacuation, including policy and procedure manual

 provided in Simulation/laptop/internet/pen/paper

Safety If at any time during the learning and assessment process, your Teacher/Assessor considers that the safety of any person is at risk they will abort the session.
Reasonable Adjustment

Students identifying as having a disability may access reasonable adjustment to enable them to participate in training and assessment on the same basis as other students. Special Consideration Where a student anticipates that a result will be impacted by special circumstances such as ill health or bereavement, application can be made for special consideration. Forms can be accessed from the Student Life office at Elgar Campus in B3.224 or download the special consideration application form
Assessment Review

If you believe that you have been assessed unfairly, you have the right to request an informal assessment review. When your assessment cover sheet is returned to you, you can request an informal review by filling in the section below and returning it to your assessor or the Operations Manager of your area. If you are not satisfied with the outcome, you can apply for a formal review of assessment on the application form located on the Box Hill Institute Website.
